Housing

  • Permanent Supportive Housing – Long-term, community-based housing, which includes supportive services for homeless people with disabilities
  • Affordable Housing Program – Generally defined as housing on which the occupant is paying no more than 30 percent of gross income for housing costs, including utilities
  • Rapid Re-Housing Program – An intervention that quickly connects families and individuals experiencing homelessness to permanent housing

Services

  • PATH Outreach – Outreach Specialists identify and engage people who are experiencing homelessness
  • Workforce Development – Help people with serious mental health conditions enter and succeed in the competitive labor market
  • Supportive Services – Housing Support Specialists provide supportive services for people experiencing homelessness, who have disabilities and reside in permanent supportive housing
