The Stark County NET program is available to anyone with a Medicaid card who is able to get themselves into a vehicle independently or with help from a caregiver. These free and discounted rides are used for trips to and from approved medical appointments but require notice at least five days in advance.
To apply for NET transportation assistance, you can reach out by phone, fax or in person. During the application process, you will be asked to provide information about yourself and your medical providers. Missing or incomplete information could result in trip delays.
If you qualify and choose to provide your own transportation, you can apply for mileage repayment. Simply call to speak with a NET coordinator and request a mileage reimbursement form to be mailed to you. With your completed form, you should submit proof of a valid driver’s license and insurance for the person who will be driving.
